Q.What forms of insurance do you accept?

We currently participate in Medicare, most commercial PPO plans and a limited number of HMO and Medicaid plans. Since this is a dynamic situation which changes periodically, we encourage you to call our office to confirm our participation in your insurance plan.

Q.What procedures do we need to be aware of as a new patient to your practice?

At your first visit, you will be required to fill out and sign certain forms which authorize us to care for you or your loved one. There will also be basic questions about your health which you will be asked to fill out on a form. Please be sure to bring a picture ID & your insurance card at all your visits. Please note, payments are due at the time of service including deductibles and co-payments.

Q.Are the doctors available after hours?

Our office is open Mon-Fri from 9AM-5PM on most days. We are always staffed with a physician during those hours to help you and to see you if needed on short notice. We encourage you to call us for routine questions during those hours. After the office is closed, there is a physician on call at all times and he/she can be reached via our main line ( 773 631 0566 ) through the answering service. This method is to be used for urgent requests only. Of course, if you are experiencing a life threatening emergency, please call 911 to be brought to your nearest emergency room.

Q.Can I access my Medical Records and test results?

Most tests done in our office are available through ‘My Chart’. You will be given a password on your first visit with us to create your secure account.

Q.Will the doctors take care of me if I am in the hospital?

We are on staff at Resurrection Medical Center and Holy Family Hospital. If you require hospital services, our doctors will continue to see you throughout your hospital stay. We are also on staff at several Rehab facilities and will continue to help you through your recovery at those facilities. Please refer to Our Providers page above for a detailed listing of facilities.
